Leads form Builder type

Leads forms are available only when Store Locator → Settings → Features → Enable leads capture is enabled.

Create the form​

  1. Go to Store Locator → Builder → New → Leads form.
  2. Add fields such as Text, Email, Phone, Textarea, or choice controls.
  3. Give every stored field a unique Name (key).
  4. Mark required visitor fields as Required.
  5. Select the outer form container to configure the submit-button label and success message.
  6. Save the form.

Lead fields are submission fields; they are not mapped to Search parameters. Search-only fields do not appear in their Type selector.

Capture page context​

Add a Hidden field, give it a key, and choose Value to capture: Current page URL, Current page path, Page title, Site URL, Referrer URL, or Browser/user agent. The browser resolves the value when the visitor submits the form.

Add reCAPTCHA​

Add a reCAPTCHA field, then configure its keys under Settings → Spam protection. The field renders only when a site key exists and is verified server-side on submit.

Set the default and embed​

Select the design under Settings → Defaults → Default lead form. Embed it with the Lead form Gutenberg block, Elementor form widget, or:

Submitted entries appear under Store Locator → Leads when the module is enabled.
